Patient Information

In the interest of patient care, kindly take note of the following:

Cancellations: Two business days notice is required to cancel or reschedule an appointment. A fee may be charged for inadequate notice.

New Patients: It is recommended that all new patients complete an oral examination by the doctor prior to scheduling treatment.

Payment Policy: Payment is due when services are rendered. We accept VISA, MasterCard, American Express & Debit Cards. We do not accept any personal cheques. Financing is available and must be discussed with the Treatment Coordinator prior to treatment.

Dental Insurance: Dental Insurance is an agreement between you and your provider. We are happy to submit your claims electronically on your behalf; assignment of benefits to our doctors is not permitted. Reimbursement from your insurance provider will be forthcoming in a few days following submission.

Monday – Thursday 8:00am – 8:00pm
Friday 8:00am – 2:00pm
Saturday 9:00am – 4:00pm
